What happens when a Russian citizen visits the USA and needs to see a doctor? Does the Russian insurance help at all, or do they need some kind of travelor's insurance to help with any medical costs?

The same rules apply as if you, as a US citizen, with your insurance sponsored by your employer or purchased by yourself(which in majority of the cases doesn't provide you with any coverage overseas) will go abroad - you'd buy a travelor's insurance, right ?

So what she needs to do is to go to a travel agency and get a travelor's insurance.
I should warn you, though if something happens in the US and she needs a medical assistance SHE will be responsible for all the expenses. The insurance company will reimburse her up to the amount provided in the agreement, when she returns back to Russia.

Another option is to buy local (US) health insurance upon her arrival to the US.

what do you mean by "any tourist, not having any status" ?

if he or she is a tourist, this implies he or she is legally in the country (has valid B-1/B-2 visa), so she or he does have a status - temporary visitor and any private insurance company would be more than happy to sell him/her health insurance for the duration of the stay in the US.

if you are talking about someone, who's out of status, then first of all he or she is not really a tourist, plus I'm not sure about the consequence of this situations - if someone, who's out of status, tries to buy health insurance policy.
